The Clinical Care Coordinator serves as the first clinical point of contact, conducting assessments and managing crisis situations for behavioral health patients. They coordinate the patient journey from initial intake and admission through treatment initiation and post-treatment follow-up.

The role involves conducting psychiatric evaluations and providing evidence-based treatment for adults and older adults. Responsibilities include managing disorders via psychopharmacology and psychotherapy, and referring patients to TMS or Spravato treatments.

Manage a clinical caseload by facilitating therapy groups and developing comprehensive treatment and discharge plans. Coordinate patient engagement and care transitions within an Intensive Outpatient Program to ensure successful clinical outcomes.

Provide evidence-based psychiatric evaluations and medication treatments for adults and older adults within a virtual intensive outpatient program. Collaborate with multidisciplinary teams and outside clinicians to manage complex mental health disorders using psychopharmacology and neuromodulation techniques.

The Clinical Case Manager ensures seamless, person-centered care for participants in Intensive Outpatient Programs through patient engagement and care coordination. Key duties include conducting orientations, leading discharge planning, and co-facilitating psychotherapy groups.
