The Renal Care Coordinator provides telephonic support and education to late-stage chronic kidney disease patients to facilitate informed decision-making regarding treatments. They act as a liaison between patients and nephrology practices to ensure comprehensive care coordination and a stable transition to renal replacement therapy.

Act as the first point of contact for patients and providers by handling high-volume inbound calls and general inquiries. Collaborate with internal clinical and non-clinical teams to ensure seamless care delivery and first-call resolutions.

Lead and coach a team of RN Care Managers to deliver coordinated virtual care and targeted interventions for kidney care patients. Monitor workflows, conduct quality assurance reviews of clinician calls, and collaborate with stakeholders to improve clinical standards and patient experience.

The Enrollment Coordinator is responsible for making outbound calls to eligible health plan members to educate them on the CKD program and persuade them to enroll. They must meet enrollment goals while ensuring compliance with company and regulatory guidelines.

Develop and deliver end-to-end machine learning solutions, encompassing technical requirement definition, scalable system architecture, and implementing monitoring and maintenance workflows. Collaborate with cross-functional teams to build new ML products and lead the design and implementation of MLOps frameworks.

The Staff Data Engineer will serve as a senior technical leader responsible for shaping, scaling, and governing the modern data ecosystem, focusing on architecture, hands-on engineering, and platform leadership to deliver high-quality data products. Key duties involve designing and implementing scalable batch and streaming data pipelines for various healthcare sources while ensuring HIPAA compliance and mentoring other engineers.
