Provide remote counseling to members via text, video, and phone to monitor progress and offer therapeutic strategies for addiction recovery. Conduct multidimensional assessments and collaborate with cross-disciplinary teams while utilizing AI-native EHR tools for documentation and clinical guidance.

The counselor will conduct clinical intakes, develop individualized treatment plans, and deliver evidence-based CBT and Motivational Interviewing via telehealth. They will also monitor member outcomes, maintain clinical documentation, and coordinate care for co-occurring conditions.

The Care Specialist serves as the primary clinical point of contact, conducting intake assessments and welcome calls to onboard members into the Pelago care model. They also maintain a small ongoing caseload, providing individual counseling and supporting clinical operations as needed.

The Medical Assistant will manage medication-assisted treatment workflows, including refills and prior authorizations, while providing compassionate support to members. They will collaborate with cross-functional care teams to ensure seamless care navigation and accurate clinical documentation.
