The Nurse Practitioner will perform Comprehensive Health Evaluations to manage comorbid conditions and reduce hospitalizations for patients with kidney disease. They will lead clinical teams in coordinating care and managing disease progression for CKD and ESKD patients.

Perform clinical chart reviews to ensure documentation accuracy and apply clinical criteria for diagnosing medical conditions. Collaborate with providers to improve documentation quality through the query process and deliver educational sessions to clinicians and coders.

The Senior Financial Analyst will build complex valuation models and manage the due diligence process for acquisitions, divestitures, and joint ventures. They will also collaborate with cross-functional teams to evaluate deals and prepare presentations for senior management and the board of directors.
