Who We Are

Vigilance Health partners with FQHCs, ACOs, IPAs, and payers to deliver Population Health and Care Management services that actually move the needle. We run Care Management, APCM, Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs — and we measure success the way our partners do: quality metrics, patient engagement, and outcomes that show up in data.

The Role

As our Quality Improvement Project Manager for ACO & Population Health, you'll be the bridge between Vigilance and its partners — the subject matter expert partners turn to for population health strategy, ACO quality measures, and quality improvement execution. You'll use ACG to stratify our patient populations, identify opportunities to improve outcomes and scores, and turn those opportunities into action alongside partner QI teams and our internal clinical leadership.

You'll also be Vigilance's ACG subject matter expert — the person who can make comprehensive patient stratification make sense to a room full of stakeholders and who tells the success stories that prove its value. Beyond service excellence, you'll actively look for ways to grow the relationship: expanding the patient panels we manage and helping partners see new services worth launching.

What You'll Own

Partner Relationship Management

You'll build and maintain relationships with every partner QI team and Vigilance-facing representative, becoming the face of Vigilance they turn to first. Trust is the foundation everything else gets built on.

ACG & Population Stratification

You'll go deep into ACG and the Vigilance Care Portal to understand the data, technology, and reporting behind our patient populations — for individual partners and Vigilance as a whole — and use that knowledge to find opportunities others miss.

Quality Measure Reporting & Analytics

You'll generate and maintain baseline reports on ACO quality measures for every partner and for Vigilance overall, giving everyone a clear, current view of program effectiveness and where the opportunities are.

Quality Improvement Strategy & Execution

You'll develop data-driven strategies for partners and targeted populations, then work with the Clinical Director to translate them into real changes — to workflows, trainings, and interventions that move outcomes and scores.

Business Development & Program Expansion

You'll keep a continuous read on partner populations, emerging challenges, and shifting regulations, using what you learn to identify new services worth launching, drive agreement renewals, and expand the patient panels Vigilance serves.

What Good Looks Like in 12 Months

• You have strong, trusted relationships with every partner QI team and Vigilance-facing representative in the network.
• You're fluent in ACG and the Vigilance Care Portal, with baseline ACO quality reports established and tracked for every partner.
• Data-driven strategies are in motion for partner and targeted populations, and the Clinical Director is executing on the workflow, training, and intervention changes you identified.
• Partners and internal teams see you as the go-to expert on ACO quality measures and population health stratification.
• A repeatable system is in place for tracking partner populations, challenges, and regulatory shifts to surface new opportunities.
• You've contributed to at least one agreement renewal or service line expansion by showing partners the value Vigilance delivers.

What You Bring

Experience That Matters Most

• Active RN license (required)
• Subject matter expertise in ACO quality measures and population health quality improvement strategies
• Experience with Johns Hopkins ACG platform or a comparable risk-stratification / population health analytics platform
• A track record of building trust and driving results with external partners — comfortable holding your own with CMOs and clinical leadership
• Strong data instincts — you're comfortable digging into a data set to find the story and the opportunity
• 5+ years of clinical experience, with meaningful time in population health, quality improvement, or ACO operations

Nice to Have

• CPHQ or a population health certification
• Experience evangelizing a technology platform or leading change management with clinical or partner audiences
• Experience growing a partner relationship — expanding patient panels or launching new service lines
