What We Need  

The Manager, Rental Census will report directly to the Director, Revenue Cycle Management and will lead a newly formed Rental Management team dedicated exclusively to Synapse Health’s active rental population of 100,000+ patients. This role is the single accountable owner for rental census accuracy, active, adherence, inactive, and held status, and for the go-forward requalification, adherence, and engagement programs that keep that census clean.

This team is the execution arm of a broader, finance-led initiative to build a rental census tool that projects monthly financial performance and tracks RightCare progress. The Manager owns the clean, actionable data that reporting layer is built on, and partners closely with Finance, RCM, Clinical Programs, and Analytics to translate census accuracy into measurable cost avoidance across high-cost rental categories, including NPWT, oxygen, and ventilators.

What You Will Do 

• Rental Census Ownership: Own the accuracy and integrity of the active rental census across all rental categories, driving the initial cleanup effort and establishing the ongoing controls that keep active rental status reconciled to paid rental status.
• Requalification & Adherence Management: Build and manage the go-forward requalification calendar, ensuring oxygen requalification is completed 30 days prior to expiration for 100% of the eligible population, and that adherence requirements are tracked and acted on before they lapse.
• High-Cost Rental Engagement: Lead active patient engagement for high-cost rentals, including NPWT outreach 7-14 days prior to rental renewal and rolling oxygen engagement tied to pick-up and cost-avoidance campaigns.
• Eligibility & Lifecycle Management: Manage ongoing eligibility checks on active rentals, including hospice, patient deceased, and mid-rental payor change scenarios, and drive timely status changes and decommission of rentals at the end of the cap period.
• Hold & Exception Management: Own rental hold management, including PAP compliance holds, and manage address change, relocation, and travel oxygen impacts on active rentals where payor jurisdiction or supplier network changes.
• Recertification Confirmation: Own confirmation of CMN and physician order recertification for PAP, oxygen, NPWT, and other items requiring renewed medical necessity, partnering with the documentation management team that owns the chase.
• Monthly Outreach Programs: Build and run recurring rental outreach programs, oxygen campaigns including VOC and hospital discharge, ventilator pickups in partnership with Clinical Programs, NPWT pre-month check-ins, and Infusystem rental management, and continuously identify new target populations and engagement strategies.
• Team Leadership: Hire, train, and develop a team of five front-line team members; set daily and weekly production expectations, build the staffing model, and protect the team’s scope so rental work is not deprioritized against competing org objectives.
• Data Quality & Reporting Partnership: Deliver the clean census data required to produce a weekly and daily view of rental adds and drops; partner with Finance, RCM, and Analytics to validate the data feeding the rental census tool and its financial projections.
• SOP & Process Development: Author and maintain the SOPs, scripting, escalation paths, and RACI boundaries for the rental management function, including clear hand-offs to Order Intake, RCM, Service, and the COE.
• RightCare Execution: Drive execution of RightCare initiatives across rental populations, quantifying cost avoidance per confirmed pickup, exhausted rental, and renewal-cycle catch, and reporting run-rate impact to leadership.

What You Have

• 5+ years of DME, HME, or home health operations experience, including 2+ years leading a front-line team.
• Working knowledge of rental billing mechanics, Medicare capped rental, cap period start and end, requalification and recertification requirements, CMNs, PAP compliance rules, and payor-specific rental rulesets.
• Hands-on experience managing an active rental or census population at scale, with demonstrated results improving data accuracy or reducing non-utilized rental cost.
• Experience building outbound patient engagement or outreach programs, including call scripting, cadence design, and production management.
• Strong analytical skills; comfortable working in large data sets, building and maintaining trackers, and partnering with Finance and Analytics on reporting definitions.
• Proven ability to write clear SOPs and stand up new processes in an environment where the process does not yet exist.
• Excellent cross-functional communication skills, with the ability to hold partner teams accountable to hand-offs and service levels.
• Proficiency with operational and workflow platforms (e.g., Salesforce, Brightree, or similar DME systems) and with Excel or equivalent.

Compensation

The base salary range for this position is $83,200 – $104,000 annually. This range reflects the anticipated compensation at the time of posting. Final compensation will be based on factors such as relevant experience, demonstrated skills, internal equity, and prevailing market conditions. Compensation discussions are encouraged throughout the hiring process.
