They will work collaboratively with FQHCs and physicians to monitor performance, conduct chart audits, and facilitate data-driven process improvements.

The Practice Transformation Consultant will implement evidence-based care management and practice transformation strategies to improve patient care, working with FQHCs and providers to meet quality objectives through data analysis and rapid cycle quality improvement methods. Essential functions include implementing strategies like QI dashboards, empanelment, and care coordination protocols, while assisting and monitoring practices in their QI initiatives.
