Manage inbound and outbound calls for a multi-state nurse triage service, assess patient needs using established protocols, and document interactions in electronic health records or the Call 4 Health platform. Support providers through medication refill coordination, critical-result notifications, referrals, follow-up care, remote patient monitoring, chronic disease education, and clinical trial screening.
