The SVP of Sales will carry an individual quota for health system deals while simultaneously mentoring and developing the sales team. They will partner with executive leadership to refine go-to-market strategies, manage pipeline accuracy, and establish scalable sales processes.
About Care Continuity
Care Continuity is redefining patient navigation. We combine clinical expertise, AI-driven insights, and compassionate human support to ensure patients receive the care they need when and where they need it.
Our solutions empower health systems and providers to close care gaps, reduce readmissions, and drive ROI through smarter, more connected navigation.
Our work is transforming how care decisions are made and we're just getting started.
About the Role
As SVP of Sales, you'll operate as both a top individual contributor and a team leader. You'll carry your own quota, closing strategic health system deals, while also mentoring and developing the broader sales team to hit theirs. This is a hands-on leadership role for someone who wants to stay close to the deal, not just manage a pipeline from a distance.
You'll partner with executive leadership to shape go-to-market strategy, refine messaging, and build the sales processes that let Care Continuity scale. At the same time, you'll be in the room for key prospect calls, helping close the deals that matter most and showing the team what excellence looks like in practice.
The ideal candidate is comfortable running their own book of business and coaching others to run theirs, someone who leads by example rather than from the sidelines.
