An exciting job role has recently become available at the Cambridgeshire. They are looking for a new member for their Transfer of Care Team.

Responsibilities:

  • Assess individuals in hospitals for care packages or care home placements under the Care Act.
  • Conduct Mental Capacity Assessments for care and support.
  • Collaborate with multidisciplinary teams to create care plans.
  • Engage in carers conversations.
  • Facilitate safeguarding discussions, ensuring a personal approach.

What we are looking for in a Social Worker:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
