Coordinate rehabilitation journeys by managing referrals from insurers and employers and arranging treatment through a supplier network. Liaise with various stakeholders to ensure clients receive efficient and accurate healthcare services.

The Specialist Case Manager will manage a caseload of rehabilitation clients, coordinating multi-disciplinary interventions and ensuring effective outcomes. They will assess client needs and develop rehabilitation or care packages while maintaining communication with stakeholders.
