Coordinate rehabilitation, care, and support for children and young people with complex clinical needs to improve their quality of life. Responsibilities include conducting assessments, managing care teams, and producing detailed formal reports for the Care Quality Commission and courts.

The Specialist Case Manager will manage a caseload of rehabilitation clients, coordinating multi-disciplinary interventions and ensuring effective outcomes. They will assess client needs and develop rehabilitation or care packages while maintaining communication with stakeholders.
