RN Case Manager
The RN Case Manager will provide clinical leadership for Managed Care, Bundle Payment, and ACO programs while developing comprehensive care plans to improve patient outcomes. They will also act as an advocate for patients and staff, overseeing data collection and facilitating smooth transitions from inpatient care.
