What We’re Looking For
Trillium Health Resources has a career opening for a Supervisor, Care Management - Innovations Waiver (IW) to join our Care Management team. This position’s primary responsibilities include overseeing a team of professionals who provide localized support within a managed care environment to members who receive Innovations Waiver funding and who have Intellectual or Developmental Disabilities or Traumatic Brain Injury. The Supervisor, Care Management - IW ensures staff implement industry standard, evidence-based and best practices to support members using the Trillium Connections Care Management platform. They provide oversight to ensure members receive whole-person care that focuses on all members’ needs including physical health, behavioral health, and social determinants of health/unmet health resource needs that help to establish positive outcomes for members.
