Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Community team, based in Bournemouth, Christchurch and Poole. This is an excellent opportunity for a dedicated practitioner to support adults with a range of long-term conditions and complex care and support needs, promoting independence, wellbeing, and positive outcomes through strengths-based and person-centred social work practice.
The successful candidate will manage a varied community-based caseload, undertaking statutory assessments, reviews, safeguarding interventions, and support planning. Working closely with service users, carers, families, health professionals, and partner agencies, you will play a key role in ensuring individuals receive appropriate support to remain safe, independent, and connected to their local communities.
This role requires a confident and experienced Social Worker with strong assessment skills, safeguarding knowledge, and experience of Court of Protection work, alongside the ability to manage complex casework involving self-neglect, risk management, and long-term health conditions.
