Job Overview

Fairview is looking to hire a Coder 4 to join our Inpatient Coding team. This is a full-time position (80 hours per pay period) and is benefit eligible. This role offers the flexibility to perform work in a virtual environment while remaining closely connected with the coding team and other partners across the organization.

 

The ideal candidate will have strong experience in hospital-based inpatient coding, with demonstrated expertise in ICD-10-CM and ICD-10-PCS coding, including accurate MS-DRG assignment. This role utilizes encoder and computer-assisted coding (CAC) software to ensure accurate, thorough, and compliant coding of complex inpatient cases. The Coder 4 will also research complex coding scenarios, apply coding guidelines and clinical documentation, and query physicians when clarification is needed to support accurate code assignment.

 

This is an HB Inpatient coding position for an experienced, trained inpatient coder.  An Inpatient Coder  analyzes clinical documentation; assigns appropriate diagnosis, procedure, and abstracts the codes and other clinical data.  This information is then used to determine reimbursement levels, assess quality of care, study patterns of illness and injuries, compare healthcare data between facilities and between physicians, and meet regulatory and payer reporting requirements. Assist in the resolution of clinical documentation and provide feedback to providers on the quality of their documentation.

 

Responsibilities:

  • Reviews and codes inpatient clinical records using ICD-10-CM and ICD-10-PCS in compliance with coding guidelines, MS-DRG/APR-DRG reimbursement rules, and Fairview standards.

  • Identifies and resolves clinical documentation discrepancies to ensure accuracy in severity, risk of mortality (SOI/ROM), Hospital Acquired Conditions (HAC), Present on Admission (POA) indicators, and data integrity.

  • Initiates and follows up on provider queries in collaboration with Clinical Documentation Integrity (CDI) staff to clarify documentation and improve code specificity.

  • Validates computer-assisted coding (CAC) output for accuracy and contributes to timely and accurate record processing to support efficient revenue cycle operations.

  • Demonstrates deep understanding of disease processes, diagnostic indicators, medications, and lab results to support accurate code assignment and quality reporting.

  • Assists in education and training of physicians and multidisciplinary staff regarding documentation and compliance with evolving coding regulations and standards.

  • Collaborates with revenue cycle team members to ensure timely claim submission and contributes to financial outcomes through accurate charge capture and documentation.

 

Required Qualifications:

  • Certificate program in coding or associate degree in HIM
  • 3+ years of coding experience
  • One of the following: Registered Health Information Administrator (RHIA) Registered Health Information Technician (RHIT), Certified Coding Specialist (CCS), Certified Inpatient Coder (CIC)
  • Basic knowledge of Windows-based computer software. Due to differences in scope of care, practice, or service across settings, the specific experience required for this position may vary.  
  • Keeping up to date with CEU’s.

Preferred Qualifications:

  • Associates or bachelor’s degree in HIM
  • 2 or more years of HB Inpatient coding experience
