Work Arrangement: Must be a current resident of Ohio

Responsibilities:

  • Supports the daily operations of the integrated care management program interventions

  • Performs in a contact center environment, effectively processing calls from Members, Providers, and other areas, internal and external to the company

  • Identifies members with Care Gaps/HEDIS-related health conditions and assists them in accessing care through Plan benefits and community resources

  • Under the direction of Clinical staff, the Care Connector will provide Members with educational materials and carry out strategies to increase healthcare adherence and reduce barriers to care

Education/Experience:

  • High School Diploma or equivalent
  • Minimum two (2) years of current/recent work experience in a customer service environment
  • Member service or customer service telephone experience desired, preferably within a managed care organization
  • Medical terminology experience preferred
  • Strongly prefer Medical Assistant, Home Health Aide, Nursing Assistant, and/or other similar health care para-professional training or certification
  • Excellent interpersonal and verbal communication skills
  • Ability to write clear and concise reports
  • Attention to detail and confidentiality with strong organizational skills

Skills & Abilities:

  • Proficient PC skills in a Windows-based environment, including MS Office products, spreadsheets, and database programs.
