Conduct telehealth physical therapy evaluations, reassessments, and treatments, and develop individualized care plans focused on residents’ mobility, function, safety, and independence. Collaborate with residents, families, on-site staff, and the interdisciplinary care team; supervise physical therapist assistants as permitted; and document care and participate in discharge planning.
