The Outreach Specialist will serve as the first point of contact for cancer survivors, conducting outbound calls to engage them and guide them through the onboarding process. They will also provide administrative support, manage patient communications, and coordinate with the care team to ensure a seamless experience.

The clinician will conduct thorough assessments, deliver evidence-based individualized and group survivorship therapy protocols, and collaborate closely with the multidisciplinary team to address patients' mental health needs within the context of their cancer survivorship journey. Responsibilities also include supporting the implementation of a collaborative care program and providing psychoeducation to survivors and their families.

As an Oncology Survivorship Nurse Practitioner, you will conduct comprehensive assessments of cancer survivors and develop personalized treatment plans. You will coordinate care among a multidisciplinary team and provide education and support to patients regarding their survivorship needs.
