Provide one-on-one therapeutic sessions and crisis follow-up services to youth and families experiencing behavioral health crises. Facilitate communication between the CAT team and external agencies while completing required clinical documentation and safety plans.

Provide resources and support to individuals with intellectual and developmental disabilities to promote independence and community inclusion. Responsibilities include performing assessments, developing person-centered plans, and coordinating with families and interdisciplinary teams.
