The Healthcare Advocate will develop trusting relationships with patients to assess their medical, social, and environmental needs while creating individualized care plans. They will also support patients by connecting them to local services, managing medical records, and coaching them on navigating healthcare complexities.

Conduct telehealth intake evaluations for older adults and collaborate with care advocates to manage patient health plans. Work with the technology team to develop AI tools that enhance the clinical experience and patient outcomes.
