The RN Care Manager oversees the development and continuity of patient plans of care while serving as a liaison between field staff and interdisciplinary teams. They manage healthcare costs through utilization management and ensure compliance with regulatory and governmental standards.

Responsible for the clinical review and summary of patient intake documentation to identify referral reasons and relay information to field clinicians. Ensures accuracy in ICD-10 coding and validates face-to-face visit documentation for insurance compliance.

Architect, design, and develop integration workflows for real-time and batch data exchange between health system partners. Lead technical discussions with stakeholders and mentor Integration/ETL engineers to ensure scalable and efficient solutions.

The RN will provide skilled nursing care in a patient's home as directed by the attending physician and will support a team of nurses by responding to after-hours patient needs. Responsibilities include observing and monitoring patient conditions, administering medication, and educating patients and families.
