As an Intake Specialist, you will play an important role in helping patients access home health services. You will process incoming referrals, verify patient and insurance information, obtain required documentation, and collaborate with hospitals, providers, referral sources, Sales, and Operations to help ensure a smooth transition into care.

Your Role as an Intake Specialist

The Intake Specialist serves as one of the first steps in the patient care process. You will be responsible for accurately processing referrals and ensuring the Intake team has the information and documentation needed to determine whether a patient can be accepted for home health services.

 

Responsibilities Include

Referral Processing

  • Receive and process incoming patient referrals from hospitals, providers, and other referral sources
  • Review referrals for completeness and accuracy
  • Ensure required orders and supporting documentation have been received
  • Follow up with referral sources to obtain missing information

Patient & Insurance Information

  • Enter and maintain accurate patient demographics, insurance, and clinical information within the EMR
  • Complete insurance eligibility and benefit verification
  • Review and reconcile insurance changes on a routine basis
  • Maintain accurate and organized referral records

Hospital & Referral Portal Navigation

  • Navigate electronic referral portals and healthcare systems
  • Process referrals through hospital and provider platforms
  • Communicate with hospital case managers and discharge planners when additional information is needed
  • Monitor incoming referrals and prioritize tasks based on urgency

Communication & Collaboration

  • Communicate professionally with referral sources, providers, patients, Sales, and Operations
  • Work closely with internal teams to support timely referral processing
  • Help resolve documentation or information discrepancies

Compliance & Confidentiality

  • Maintain patient information in accordance with HIPAA and company policies
  • Handle confidential medical and insurance information with accuracy and discretion
  • Ensure documentation is complete, organized, and properly maintained

 

Technology You May Work With

Our Intake team works with multiple electronic systems and referral platforms. Experience with any of the following is helpful:

  • EMR platforms
  • Aiden
  • naviHealth
  • CarePort
  • Mayo Clinic referral portals
  • Insurance verification and eligibility portals
  • Other hospital and healthcare referral systems


Experience with multiple systems is preferred, but we provide training on our internal processes and platforms.

 

Qualifications

Required

  • At least 1 year of recent experience in medical intake, referral processing, healthcare administration, or a similar healthcare environment
  • Experience navigating EMR systems
  • Experience with medical referrals and documentation
  • Experience with insurance verification and eligibility
  • Strong attention to detail and accuracy
  • Ability to manage a high volume of information while maintaining accuracy
  • Strong organizational and multitasking skills
  • Excellent written and verbal communication skills
  • Strong customer service skills
  • Ability to prioritize work based on urgency and patient needs
  • Florida residency required
  • Ability to maintain a quiet, professional, HIPAA-compliant home workspace
  • Reliable internet connection and ability to work independently in a remote environment

Preferred

  • Experience working with hospital referral portals such as Aiden, naviHealth, CarePort, Mayo, or similar platforms
  • Experience working in a home health or healthcare Intake department
  • Experience with multiple EMR platforms
  • Clinical or healthcare background such as Medical Assistant, CNA, or similar
  • Experience working in a high-volume healthcare environment
