Position Summary

The Supportive Services for Veteran Families (SSVF) Case Manager provides direct case management and housing stabilization services to eligible Veterans and Veteran families experiencing homelessness or who are at imminent risk of homelessness. Based in Juneau, Alaska, this position serves Veterans throughout Southeast Alaska by connecting participants with housing, benefits, healthcare, employment, and other supportive services that promote long-term housing stability and self-sufficiency.

 

Although located in Juneau, the Case Manager reports to the Fairbanks Rescue Mission headquarters in Fairbanks, Alaska, and works closely with the Veterans Department, the U.S. Department of Veterans Affairs, landlords, and community partners throughout the region. This position requires independent judgment, strong organizational skills, and the ability to work remotely while maintaining compliance with SSVF program requirements and Fairbanks Rescue Mission policies.



Essential Duties and Responsibilities

  • Conduct participant eligibility screenings, assessments, and ongoing case management.
  • Develop individualized housing stabilization plans and monitor participant progress toward self-sufficiency.
  • Assist Veterans and their families in obtaining and maintaining safe, stable, and affordable housing.
  • Coordinate and provide supportive services, including VA benefits navigation, employment assistance, healthcare and behavioral health referrals, substance use treatment referrals, financial education, transportation, and other community resources.
  • Conduct outreach and build collaborative relationships with landlords, property managers, healthcare providers, employers, community agencies, and other service organizations throughout Southeast Alaska.
  • Coordinate temporary financial assistance in accordance with SSVF program guidelines.
  • Complete program intake, assessments, discharge planning, follow-up services, and aftercare support.
  • Maintain accurate, timely, and confidential participant records in HMIS, VA systems, and agency databases.
  • Ensure compliance with Department of Veterans Affairs, SSVF, agency, and grant requirements.
  • Participate in coordinated entry, case conferencing, staff meetings, trainings, and professional development opportunities.
  • Provide regular communication and updates to the Director of Supportive Services while collaborating with Fairbanks Rescue Mission staff.
  • Demonstrate professionalism, compassion, cultural sensitivity, and respect in all interactions with Veterans, families, and community partners.
  • Perform other duties as assigned.



Qualifications

Required

  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field; equivalent education and experience may be considered.
  • One (1) year or more of experience providing case management, housing assistance, or related human services.
  • Knowledge of trauma-informed care, strengths-based case management, and crisis intervention.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to work independently while managing multiple priorities.
  • Proficiency in Microsoft Office and electronic case management systems.
  • Valid Alaska Driver's License, reliable transportation, and the ability to travel throughout Southeast Alaska.
  • Ability to successfully pass required background checks.

Preferred

  • Experience working with Veterans or military families.
  • Familiarity with VA, HUD, SSVF, or homeless assistance programs.
  • Experience using HMIS or comparable case management software.
  • Knowledge of Alaska housing resources and community services.

Physical Requirements

  • Ability to sit, stand, walk, bend, and lift up to 40 pounds.
  • Ability to conduct home visits and community outreach in various environments.
  • Ability to travel throughout Southeast Alaska, with occasional travel to Fairbanks for meetings, training, and program activities.



Work Environment

This position is based in Juneau, Alaska, and provides services throughout Southeast Alaska. Work is performed in an office setting as well as in participant homes and community locations. The position requires frequent interaction with Veterans, landlords, healthcare providers, community partners, and the Department of Veterans Affairs. Standard work hours are Monday through Friday; however, occasional evening, weekend, and overnight travel may be required to meet participant and program needs. Although based remotely, the position reports to the Fairbanks Rescue Mission headquarters and maintains regular communication with supervisory staff.
