The Population Health Navigator I coordinates care and resources for patients to support population health initiatives and improve quality outcomes. They proactively manage care gaps, schedule appointments, and assist patients in navigating the healthcare system and insurance benefits.

As a Population Health Navigator I, you play a key role in supporting population health initiatives by coordinating care and resources in collaboration with Primary Care Providers and the healthcare team. Using a comprehensive, holistic, proactive and “Patients First” approach, you will help patients and their care partners navigate the Cleveland Clinic Health System, schedule appointments, connect to community resources and address health-related needs.
A caregiver in this role works remotely from 8:00 a.m. – 4:30 p.m. Candidates must reside in Ohio and be available to attend in person for equipment pickup and any required on-site meetings.
