You will work as a variable service provider NP/PA (APP) role with an average of 20-30 hours per week for a 6-month period. You will work in a remote environment, reporting to our Associate Director, Advanced practice Provider. There are no benefits tied to this position.
You will spend your day performing the following duties:
- Conducting virtual (audio and video) visits with CenterWell patients to care for chronic conditions, discuss open care gaps, and connect patients back to an in-person PCP.
- Address acute and chronic conditions and preventive care needs. Creating and registering, and documenting a patient record within Athena
- Verify benefits
- Comprehensive patient evaluation including medication and medical history review. Coordinating management of members social needs to help facilitate gap closure – e.g., providing transportation services
- Ensure adequate follow up regarding receipt of results, medication initiation, and PCP follow up

You will excel in an integrated, team-based care environment, adapt to value-based care practices, and remain focused on delivering high-quality outcomes. Required Qualifications:
- You will have a Master's degree or higher
- You will hold a certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners Certification Board (AANPCB
- You will have an active, unrestricted NP license in 2-3 or more of the following states: FL, TX, AZ, GA, LA, NC, SC, MS, VA prior to the start of employment
Preferred Qualifications:
- Previous experience in a value-based care or Medicare Advantage setting
- Knowledge of HEDIS measures and quality indicators
- Experience conducting annual wellness or developing comprehensive care plans for patients who may only be seen once
- Experience in virtual care and remote work
