Comagine Health is seeking an Improvement Advisor to support planning, implementation, and operational coordination of the Utah Community Care Hub (CCH). In this role, you will collaborate with project teams, health care providers, local health departments, community-based organizations, and other partners to support initiatives that improve care coordination and health outcomes. Working with community-based organizations is a top priority.

The Improvement Advisor provides consulting and technical assistance to a variety of partners—including hospitals, practices, local health departments and community-based organizations—to support Community Care Hub implementation, care coordination, health system transformation and performance improvement initiatives in rural Utah.

Key Responsibilities

• Guide organizations through quality improvement and community health initiatives
 • Coach partners on data collection, interpretation, and performance improvement strategies
 • Design and implement evidence-based quality improvement interventions and support development and improvement of care coordination and referral workflows
 • Facilitate learning collaboratives, trainings, technical assistance, and partner engagement meetings
 • Partner with interdisciplinary teams and Community Health Workers to support care coordination, community engagement, referral workflows and community health initiatives
 • Support partner readiness, onboarding, and implementation activities with health care and community-based organizations
 • Track and document activities using CRM and project management tools

Qualifications

✔ Bachelor’s degree in public health, health care, or a related field
✔ 4+ years of health care, public health, community-based social services, or related experience, including quality improvement work
✔ 2+ years of project management experience
✔ Previous outpatient clinic experience preferred
✔ Strong knowledge of quality improvement methods, health care terminology, and clinical workflows
✔ Excellent communication and partner engagement skills

Preferred

Master’s degree (MA/MS) in Public Health, Nursing, Health Care Administration, Public Policy, or related field
