Preamble

Scope of work

• Performing HFDM Crew Contacts
• Validation of Flight Actions
• Regularly checking HFDM
• Produce periodic HFDM Base monthly reports (if required by HFDM Manager)

Key responsibilities

MAIN RESPONSIBILITIES

It is the responsibility of the Flight Crew Representative to contact the crew (in consultation with the Gatekeeper) when an event exceedance (significant Level 2 or Level 3) has been identified and to explain and review that event, using playback and analysis software where appropriate. The pilot liaison will also relay any comments from the crew to the HFDM Programme. The Flight Crew Representative also acts as the pilot's advocate if requested to do so by the HRG or the pilot himself. He may be present when requested to do so by the pilot during the interview resulting from HFDM finding. When it is necessary to convene the HRG for the purpose of identifying a pilot, the pilot concerned may want to appoint a Flight Crew Representative. He has the right to choose from the Company’s pilot pool. This representative will act on a voluntary basis. Travel and accommodation will be borne by the Company. This role should be filled by an experienced and trusted pilot. In Westair usually they will be a LTCC so they are able to provide technical advice.

Specific Functions

• To ensure that any significant low, medium and high-risk events are highlighted to the flight crews as soon as possible to prevent the risk of re‐occurrence. Events which require flight action need to be communicated to pilots within a period when they will be able to recall the flight with clarity and assist in HFDM Implementation and Operations Plan Weststar NDD (Westair) analysis process. The risk of delaying feedback requests is that pilots may be unable to recall particular flights and hence the benefit of the feedback system is lost. Therefore if a flight action is raised the FCR should make contact with the crew within 24 hours but (exceptionally) not exceeding 7 days.

• To make a daily check in Aerobytes to confirm the integrity of the previous day’s HFDM uploads. It is common for failed downloads and this a valuable tool for alerting the HFDM Manager that there is an upload issue with a particular airframe or HFDM ground station.

• To confirm the Flight action requested by Helinalysis is valid. FCRs must make a record of any Flight actions in the Crew Contact Diary in One Drive.

• To carry out a periodic review of de‐identified HFDM data findings as member of the HFDM Review Group (HRG).

• To determine and periodically review the alert detection threshold positions.

• Together with HFDM Team, to make recommendations for changes to procedures and training to the Training & Standard of Flight Operations Management.

• To investigate significant events discovered by the HFDM Programme.

• Together with HFDM team, to make the decision to remove the protection of confidentiality in cases of gross misconduct or continued non‐compliance with SOP's. In such cases details of the case will be brought to the HRG to decide on the action taken. In this way the HFDM Programme remains just, as opposed to non‐punitive.

• If required by the HFDM Manager, to produce periodic HFDM Base monthly reports and provide data in a format usable and easily understood for pilot debriefs and flight action or crew feedback requests.
