ESSENTIAL JOB FUNCTIONS:

1. Provides direct support services to include engaging clients in Group Therapy, topics related to 12-step recovery, health and wellness promotion, and crisis management.

2. Conducts assessments or screenings with clients to evaluate the nature of their presenting problems to determine their needs and develop appropriate treatment plan goals.

3. Develops and provides individual treatment plans; monitors, evaluates and documents client’s progress.

4. Has responsibility for intake documentation. Prepares progress notes, event notifications and other documentation regarding clients in accordance with regulatory requirements and company policies and procedures.

5. Monitors case load daily to ensure required documentation is completed timely; including, but not limited to, assessments, treatment plans, case reviews, case management sessions and discharge plans.

6. Facilitates coordination of care to include case review, therapeutic interventions, and client progress in treatment.

7. Assist/coach recovering persons develop their own plan for advancing their recovery. 
8. Facilitate the transition from professional services to community based services. 
9. Promote self-advocacy by assisting recovering persons to have their voices fully heard; their needs, goals and objectives established as the focal point of rehabilitation and clinical services. 
10. Actively identify and support linkages to community resources (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self- help groups, professional services, etc.) that support the recovering person’s goals and interests. 
11. Provide hope and encouragement for opportunities at varying levels of involvement in community based activities (e.g., work, school, relationships, physical activity, self-directed hobbies, etc.). 
12. Develop relationships with community groups/agencies in partnership with others in the agency. 
13. Identify barriers (internal and external) to full participation in community resources and develop strategies to overcome those barriers. 

14. Completes other tasks as assigned.

 

 

EDUCATION AND EXPERIENCE

·      Bachelor’s Degree in a non-related field and at least one year of full-time Behavioral Health work experience, or

·      Have an associate degree and at least two years of full-time Behavioral Health work experience. Or

·      High School Diploma or equivalent and at least three years of full-time Behavioral Health work or experience.

 

KNOWLEDGE, ABILITIES, AND ATTITUDES REQUIRED

1.     Knowledge of software programs for personal computer use, especially word processing, video conferencing, email, and spreadsheets.

2.     Knowledge and experience in counseling and case management with substance abuse clients.

3.     Ability to provide services via video conferencing/teletherapy when determined appropriate for clients and the organization. Teletherapy may include evaluation, assessment, consultation, treatment planning, case management, coaching, individual therapy, and group therapy. Telehealth will occur primarily through interactive audio, video, telephone and/or other audio/video communications.

4.     Ability to prepare, implement, and analyze treatment plans and psychiatric reports.

5.     Strong problem-solving skills and the ability to carry out assigned projects to their completion.

6.     Strong interpersonal skills, including the ability to communicate effectively both verbally and in writing, establishing and maintaining effective working relationships, gaining cooperation, and resolving conflicts.

7.     Ability to plan and organize case workload, manage deadlines of him/herself.

8.     Ability to maintain and demonstrate ethical professional standards and demeanor, including the confidentiality of employee and client data, client boundaries, and other sensitive information.

9.     Demonstrated competence with, and continued commitment to respect for cultural differences and diversity, protecting client rights.

10.  Recognition of the limits of one’s own professional expertise and the willingness to seek supervision and/or support, as needed.

11.  Makes clinical and organizational decisions with the clients’ and agency’s well-being and best interests in mind.

12.  Provide treatment that promotes client dignity, independence, individuality, strength, privacy, and choice, recognizing the unique needs of the client population served by the agency.

13.  Provide the behavioral health services that the agency is authorized to provide and that the staff member is qualified to provide.

14.  Recognize obvious symptoms of a mental disorder, personality disorder, or substance abuse.

15.  Identify types of medications commonly prescribed for mental disorders, personality disorders and substance abuse, and the common side effects and adverse reactions to medications.

16.  Provide ethical and professional services to clients and families ensuring confidentiality laws including HIPAA (Health Insurance Portability and Accountability Act) are always adhered to.

17.  Recognize, prevent, and respond to a situation in which the client may be a danger to himself or others, behaves in an aggressive or destructive manner, is experiencing a crisis or medical emergency.

18.  Recognize and respond to fire, disaster, hazard, and medical emergency: and

19.   Has completed all the requirements above and is documented in application in training including the ability to pass background checks required by Modern Recovery Services.

 

PHYSICAL DEMANDS

Must be able to walk, bend, stand, climb stairs, speak English, hear, use a telephone, drive an automobile, and operate standard office equipment, such as a computer, copier, and fax machine. Must be able to lift and carry up to 25 pounds.
