One of LSC's Child and Family communities is seeking a full-time AFL Care Coordinator to provide clinical guidance, behavioral intervention, care coordination, and support for individuals participating in our Alternative Family Living (AFL) program.
This position works closely with clients, host families, and the clinical team to promote placement stability, support behavioral growth, and help individuals achieve successful outcomes in a family-based environment.
Location
Remote/Home-Based Position
This role allows you to work remotely from your home office while serving clients throughout the assigned region. Regular travel is required for home visits, client support, and community-based services. Monthly travel throughout the service area should be expected.

Essential Responsibilities
Coordinate and monitor services for clients participating in the AFL program.
Serve as a liaison between clients, host families, providers, community resources, and support systems.
Assist with assessments, placement matching, and service coordination.
Develop, implement, and monitor behavioral intervention and treatment plans.
Provide support, coaching, and crisis intervention to clients and host families.
Participate in clinical team meetings and collaborate on treatment strategies.
Maintain documentation that meets state, federal, and agency requirements.
Complete placement paperwork, authorizations, attendance records, and billing documentation.
Utilize person-centered planning practices to support client goals and outcomes.
Assist with transition planning and discharge coordination when necessary.
Support outreach, recruitment efforts, and relationships with referral sources.
Promote Lutheran Services Carolinas within the community and human services network.

Education & Experience
One of the following is required:
Bachelor's degree in Social Work, Psychology, Human Services, or a related field and two years of related experience, including two years in client welfare services, OR
Bachelor's degree in a non-related field and five years of related experience, including two years in client welfare services, OR
Master's degree in a Human Services-related field and one year of client welfare experience.
Desired Knowledge & Skills
Knowledge of child welfare and behavioral health systems
Experience conducting assessments and developing behavioral treatment plans
Strong crisis intervention and behavior management skills
Understanding of family systems, attachment theory, psychopathology, and psychotropic medications
Knowledge of person-centered planning practices
Excellent communication, organization, and documentation skills
Ability to work independently while collaborating with a multidisciplinary team
Strong time-management and problem-solving abilities
Requirements
Must be at least 21 years of age
Valid driver's license required
Reliable transportation required
Ability to travel throughout the assigned region for home visits and client support
Flexibility to work occasional evenings, weekends, and on-call shifts as needed
Ability to lift and carry up to 25 pounds
