What We’re Looking For


Trillium Health Resources has a career opening for a Director, Care Management – Innovations Waiver to join our team! The Director, Care Management – Innovations Waiver oversees a skilled, integrated team of professionals who provide localized support within a managed care environment for members living with intellectual/developmental disabilities or traumatic brain injury. The Director ensures staff implement industry standard, evidenced-based and best practices to support members. The Director provides oversight to ensure members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health that helps to establish positive outcomes for members and transform their lives for the better.


On a typical day, you might:


  • Provide direct supervision and oversight for a localized, Integrated Care Team supporting Innovations members within designated region. funding living with mental health, substance use, and intellectual/developmental disabilities.
  • Provide support, education, and guidance to staff as needed.
  • Develop, implement, and monitor standards, procedures, and processes with approval of Operations Director and Senior Director of IDD Care Management.
  • Provide leadership with current programs or projects as assigned. May conduct research or needs assessments or attend training programs to assess opportunities for Trillium. May assist with implementation of same.
  • Review dashboard metrics, to ensure compliance with Medicaid, HCBS, and Innovations Waiver requirements.
  • Monitor critical incidents and escalated concerns and resolve member and providers concerns.

Qualifications


Required:

  • Bachelor's degree in a health or human services field and three (3) years of progressive experience in care management, case management, or care coordination with MH/SU/DD populations, including experience in program development and/or program management. 
  • Must have a valid driver’s license. 
  • Must reside within North Carolina.
  • Must be able to travel within catchment as required.


Preferred:

  • Experience supervising a team (2 or more employees) 
  • Experience focused on working with Innovations in the mental health, substance use, and/or intellectual/developmental disability and TBI populations
  • Experience in Innovations Waiver, HCBS, and Care Management, and Managed Care
