The Nurse Practitioner is responsible for the delivery of patient care through assessment, diagnosis, and treatment of both acute and chronic conditions. They also provide leadership in the practice location and engage in professional development and teaching.
Provide virtual psychosocial assessments and individual psychotherapy to adults with serious mental illness and substance use disorders. Collaborate with a multidisciplinary care team to monitor clinical symptoms and resolve social determinants of health.
The PMHNP will evaluate and diagnose patients with mental health disorders while developing and implementing individualized treatment plans. They will also collaborate with a multidisciplinary team to manage medications and monitor patient progress.
