ASSOCIATION FOR INDIVIDUAL DEVELOPMENT

The therapist is responsible for providing case management and counseling services to clients with persistent mental illness while maintaining accurate clinical documentation. They will develop individual treatment plans and promote client wellness through coaching and community-based referrals.

They are responsible for developing individual treatment plans and maintaining accurate clinical documentation in accordance with state and agency standards.

The Case Manager QIDP is responsible for the clinical case management of an assigned caseload of up to 20 clients in I/DD Service Programs, developing individualized strategies to coordinate services and maximize client progress toward outcomes and community inclusion. This role also involves maintaining comprehensive documentation of services according to regulatory requirements and participating in ongoing quality assurance measures.
