BAYADA Home Health Care is seeking a compassionate, driven, and relationship-focused Transitional Care Manager to support the ERM market. This is a non-clinical role focused on building referral relationships, connecting clients and families with home health services, and supporting successful transitions from hospitals and other health care settings to care at home.

What you’ll do

• Develop and maintain strong professional relationships, in accordance with The BAYADA Way, with our referral partners and their teams.
• Serve as a knowledgeable and trusted resource on home health services and the transition from facility-based care to care at home, and when a referral to home health directly from a home visit would positively impact outcomes and patient experience recognizing that many referrals will come from NPs whose patients are already living at home.
• Clearly articulate the features and benefits of BAYADA services to referral partners, clients, and families.
• Identify individuals who may benefit from home health care and coordinate appropriate referrals with the BAYADA service office.
• Develop a strong understanding of BAYADA’s home health services, referral process, and available resources.
• Gather and communicate referral information accurately to the appropriate BAYADA service-office and clinical teams.
• Coordinate the non-clinical aspects of the referral process and support timely communication among referral partners, clients, families, and BAYADA teams.
• Follow up with referral sources and internal partners to help ensure a smooth transition of care.
• Maintain accurate referral information, follow-up activity, and required documentation.
• Participate in market development activities and support growth through ethical, service-focused relationship building.
• Represent BAYADA professionally in hospitals, rehabilitation facilities, physician practices, community settings, and other referral environments throughout the ERM market.
• Collaborate with Transitional Care Managers, Marketing Managers, clinical leaders, and service-office teams to support clients and referral partners.

Qualifications

• This is a non-clinical position; a clinical license is not required.
• At least three years of experience in sales, marketing, community relations, health care business development, referral coordination, customer service, or a similar role.
• Previous experience working with hospitals, rehabilitation facilities, physician practices, discharge planners, or community-based health care partners preferred.
• Demonstrated record of strong interpersonal, communication, and relationship-building skills.
• Ability to build credibility and establish productive relationships with referral sources and health care professionals.
• Strong organization, follow-through, and time-management skills.
• Ability to work independently, manage priorities, and collaborate effectively across teams.
• Willingness and ability to travel regularly throughout the assigned ERM market.
