Standard working hours: 8am to 5pm EST, M-F
Location: Remote in Virginia or North Carolina.
Job Description Summary
Assumes responsibility, accountability, and leadership for the daily operations, including coordination of work, quality, and service.
First-line supervisor in the Department of Medical Care Management for assigned site/function. Facilitates the work of assigned team members.
Provides a leadership role in ongoing case manager competency assessment, needs identification, and educational offerings.
Provides educational services to the Medical Care Management staff.
Participates in the work activities of assigned teams and provides case management services as needed.
Supervise a team of up to 10 to 14 FTEs (RN and Patient Services Coordinator)
Education:
Bachelors Degree in Nursing REQUIRED
Certification/Licensure
Registered Nurse (RN) License (Compact or Virginia) preferred
For Integrated Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM or RN-BC). For other service lines, certification based on specialty area required within one year of eligibility.
Experience
3 years experience in Case Management REQUIRED
Experience working with low and high risk pregnant population (Maternal Health / Perinatal / OB-GYN /Labor and Delivery / Mother-Baby / Postpartum) experience highly preferred
Health Plan experience preferred
Supervisory experience preferred
Proficient with NCQA guidelines
Experience with managing teams in a remote environment, motivational interviewing & member engagement, and discharge planning preferred
