RN Case Manager – Transitions of Care
Remote | Full-Time | Bilingual Spanish or Vietnamese

Alignment Health is hiring a Remote RN Case Manager to support members transitioning from hospital or skilled nursing care. This role focuses on reducing readmissions, coordinating post-discharge care, and providing education and resources to patients and families—all conducted telephonically.

Schedule: Must be willing to work Mon - Fri, 8am - 5pm Pacific Time

What You’ll Do:
  • Develop and manage personalized discharge care plans
  • Coordinate services across providers and community resources
  • Educate members and families on diagnoses, medications, and self-care
  • Monitor progress and ensure continuity of care
  • Advocate for member needs and support their recovery
What You Need:
  • Active, unrestricted California RN license
  • Bilingual in Spanish or Vietnamese
  • 2+ years of case management or clinical experience
  • Willing to obtain licensure in other designated states (Non-compact: NV; Compact: AZ, NC, TX) within the first 6 months of employment (licensure fees reimbursed by the company)
  • Strong communication and care coordination skills

Knowledge
  • Knowledge of Medicare Managed Care Plans, insurance regulations and community resources
Specialized Skills:
• Required:
  • Ability to communicate positively, professionally and effectively with others; provide leadership, teach and collaborate with others.
  • Effective written and oral communication skills; ability to establish and maintain a constructive relationship with diverse members, management, employees and vendors;
  • Mathematical Skills:  Ability to perform mathematical calculations and calculate simple statistics correctly
  • Reasoning Skills:  Ability to prioritize multiple tasks; advanced problem-solving; ability to use advanced reasoning to define problems, collect data, establish facts, draw valid conclusions, and design, implement and manage appropriate resolution.
  • Problem-Solving Skills:  Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.
  • Report Analysis Skills:  Comprehend and analyze statistical reports.

Pay Range: $85,696.00 - $128,543.00

Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
