About the Role:
MedPOINT Management in Sherman Oaks, CA is looking for a dedicated Re-Admissions Case Manager to join our growing healthcare team. In this impactful role, you'll play a critical part in coordinating patient care transitions and reducing preventable hospital readmissions. Responsibilities:
- Monitor and manage high-risk patients post-discharge to prevent unnecessary hospital readmissions
- Coordinate care transitions between hospitals, skilled nursing facilities, and outpatient providers
- Conduct telephonic and in-person patient assessments to identify barriers to care and social determinants of health
- Collaborate with interdisciplinary care teams including physicians, social workers, and utilization management staff
- Develop and implement individualized care plans focused on reducing readmission risk
- Track and document case management activities in electronic health record (EHR) systems
- Educate patients and caregivers on discharge instructions, medication adherence, and follow-up care
Requirements:
- Active RN or Licensed Clinical Social Worker (LCSW) license in the state of California
- 2+ years of case management, utilization management, or discharge planning experience
- Strong knowledge of managed care, IPA, or health plan operations preferred
- Experience with care transitions, readmission reduction programs, or chronic disease management
- Proficiency with EHR/case management software and Microsoft Office Suite
- Excellent communication, critical thinking, and organizational skills
- CCM (Certified Case Manager) certification is a plus
