The Food Is Medicine San Gabriel Valley Program (FIM-SGV) will expand the Food4All SGV initiative to further reduce food insecurity and advance Food is Medicine (FIM) strategies for food-insecure patients with chronic conditions served by six Community Health Centers (CHCs) in the San Gabriel Valley (SGV), strengthening healthcare–food organization partnerships and managing implementation of clinic-based farmers markets and a produce prescription program.
This program, which will be implemented under the leadership and guidance of the Health Consortium of Greater SGV/Heluna Health and the Greater SGV Hospital Collaborative, will deepen partnerships with CHCs serving the region’s most vulnerable populations, enhance clinic capacity to connect patients with nutritious food, and support long-term sustainability.
Reporting to the Health Consortium Director, the Program Manager will manage program implementation across six clinics, coordinating multiple partners, evaluation activities, and technical assistance. The Program Manager will collaborate with program partners to support the development of the program infrastructure and ensure effective implementation of program objectives. Their efforts will also include sustaining the existing Food4All SGV program.
This is a part time, non-benefitted position at 20 hours per week, possibly expanding to 24 hours per week in November 2026 pending additional funding. This position is grant-funded through July 2028 with possible extension pending funding availability.
Pay rate is $35 hourly
This position will require occasional local travel in the San Gabriel Valley area of Los Angeles County.

KEY RESPONSIBILITIES
Program Management & Implementation
- Coordinate the day-to-day implementation of the FIM-SGV program across participating community health centers.
- Develop and maintain work plans, implementation timelines, and tracking systems to ensure program objectives and grant deliverables are met.
- Coordinate program activities with clinic staff, Seeds of Hope, the evaluation consultant, and with the Technical Assistance provider.
- Monitor project milestones and identify barriers to successful implementation, recommending solutions as needed.
Clinic Engagement & Partnership Development
- Serve as the primary liaison with participating clinics and community partners.
- Coordinate onboarding of participating clinics, including orientation meetings and implementation planning.
- Build and maintain strong relationships with clinic leads and frontline staff.
- Facilitate regular communication among project partners to ensure effective collaboration.
- Organize and facilitate workgroup meetings, technical assistance sessions, and partner meetings.
Farmers Market Coordination
- Coordinate implementation of bi-weekly Free Farmers Markets at participating clinic sites in partnership with Seeds of Hope.
- Work with clinics and Seeds of Hope to develop market schedules and operational plans.
- Monitor market operations and participant experience to ensure consistent program quality.
- Track participation, attendance, and operational metrics for each market.
Produce Prescription Program Coordination
- Coordinate implementation of the Produce Prescription program with participating clinics.
- Work with clinic staff to identify and enroll eligible patients.
- Coordinate with Seeds of Hope regarding produce box distribution schedules and participant engagement.
- Support scheduling and promotion of onsite cooking demonstrations/classes.
- Monitor and document participant retention and program completion.
Communications & Outreach
- Coordinate development of promotional materials for clinics and support clinics in promoting Farmers Markets through outreach materials, social media, patient communications, and other marketing efforts.
- Assist partners with outreach strategies to increase patient participation.
- Maintain rosters of participant contacts and regularly communicate with participating organizations regarding program updates and activities.
- Document success stories and program accomplishments for reports, presentations, and communications.
Data Collection, Evaluation & Reporting
- Maintain accurate program records and databases.
- Track program performance indicators, including:
- Farmers Markets conducted
- Participant attendance
- Unduplicated participants
- Produce boxes distributed
- Patient enrollment and completion
- Technical assistance activities
- Cooking classes delivered
- Coordinate collection of participant surveys and other evaluation data.
- Work with clinic staff and evaluation partners to monitor clinical outcome measures, including A1C and blood pressure improvements.
- Prepare regular reports summarizing program activities, outcomes, and grant performance.
- Assist with preparation of funder reports and presentations.
Implement Workgroup Meetings & Other Administrative Responsibilities
- Maintain organized project files and documentation.
- Schedule regular workgroup meetings and prepare agendas and meeting summaries for both FIM-SGV and the SGV Hospital Collaborative.
- Track project budgets and expenses as assigned.
- Support Project Director with contract coordination, purchasing, budgeting, and other administrative processes as needed.
- Perform other duties related to successful implementation of the program.

JOB QUALIFICATIONS
Education/Experience
- Bachelor’s degree in Healthcare Management, Health Administration, Health Systems Management, Public Health, or related field preferred. Master’s degree preferred.
- Background in community health and/or healthcare systems.
- Previous experience as a program coordinator or manager working with multiple organizations.
- Experience with data management systems preferred.
Certificates/Licenses/Clearances
- Successful Background check clearance
- A valid California driver’s license, proof of vehicle insurance, and reliable transportation or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions
