The Population Health Navigator III strengthens connections between Cleveland Clinic providers and primary care practices across the network, providing resources and support to promote coordinated, high-quality care. This role supports utilization of Cleveland Clinic network providers and leverages the electronic medical record (EMR) to facilitate timely and effective communication and coordination throughout the patient’s care journey. 

Using a comprehensive, holistic, proactive, and “Patients First” approach, the Population Health Navigator III partners with patients, care partners, and primary care practices to support positive outcomes. The role also works closely with practices to build capacity for performance improvement initiatives and advance both incremental and transformative improvements in care delivery. This is an active, fast-paced role that requires strong attention to detail, organization, and the ability to effectively manage multiple priorities. 

A caregiver in this position works M-F, 7:00am—3:30pm. No weekends or holidays.

A caregiver who excels in this role will: 

• Serve as subject matter expert for the Navigation team.

• Implement new processes to guide patients to the appropriate resources and support services.

• Identify, evaluate, and resolve patient concerns.

• Participate in multi-disciplinary team projects to build competency around specific department tools and methods for organizational department growth, development, and improvement.

• Maintain departmental workflow and communications with external and internal stakeholders.

• Develop and enhance communication standards and relationships with internal providers across the Cleveland Clinic network.

• Explore opportunities to add value to job accomplishments and drive the Cleveland Clinic mission.

• Complete data tracking and collection to assist with the identification of routine statistics, opportunities, and trends across assigned patient populations and the Cleveland Clinic network.

• Review, facilitate, and complete care gap closures for assigned patient populations.

• Other duties as assigned.

• Assess and provide feedback to practices regarding organizational, clinical, and business functions to drive change for value-based operations.

• Use practice-level and physician data to drive change.

• Coordinate and implement the distribution of new clinical functionality practices.

• Travels to provider offices and schedules group user training when necessary.

• Document and diagram of existing workflows. Identifies opportunities for improvement and works with the local practice team and Navigation Leadership team to design and implement these improvements.

• Partner with local practice leadership to train practice staff to understand and use data effectively to drive Performance Improvement in the measurement domains for assigned patient population.

• Promote best practices sharing in quality structures and methods related to performance measures.

• Build a team orientation and promote effective communication patterns among practice staff and help to create a practice culture that is receptive to change.
  

Minimum qualifications for the ideal future caregiver include: 

• High School Diploma or GED

• 2 years experience as a Navigator II

• 6 years recent experience working with patients in a clinic setting, or other equivalent position, which includes interacting with patients, physicians, and other departments in a collaborative manner

• OR Associate’s degree and four years of experience

• OR   Bachelor’s degree and two years of experience

• Epic scheduling experience

• Strong patient advocacy skills

• collaborate with multiple care teams (e.g., urgent dispatch, medical care at home)

Preferred qualifications for the ideal future caregiver include: 

• Associate’s or Bachelor’s Degree

• MA background

• Experience working with geriatric populations and understanding their unique needs

• Strong problem-solving and adaptability skills

• Prior experience in a clinical office setting

Physical Requirements:

• Ability to operate a computer and other office equipment.
• Ability to travel throughout the hospital system and, in some locations, off-site.
• Ability to communicate and exchange accurate information.

Personal Protective Equipment:

• Follow standard precautions using personal protective equipment as required.
