The National Director of Patient Outcomes provides direction and leadership for ClearSky Health’s hospitals in the areas of IRF-PAI compliance, and Case Management.  The position is responsible for the development, education, and implementation of processes for the hospital DPOs and Case Managers to operate effectively and efficiently.  The position also works with other key team members to ensure quality outcomes in a cost-effective manner. This position must integrate company values into daily practice.

Essential Functions:

  • Provides education, direction and mentorship of the case management department’s function, purpose, and goals.  Areas include Team Conference, Utilization Review and Discharge Planning.
  • Leads the development, implementation, and evaluations of companywide patient care programs in relation to case management.
  • Works with hospital leaders to develop and manage a quality case management operational model according to community needs and industry standard.
  • Develops guidelines for working with insurers, managed care organizations, referral providers, patients, and families.
  • Works with hospital DPO’s on managing staffing needs and caseloads by evaluating census, acuity, skill level, and staff availability.
  • Responsible for accreditation standards to ensure all facilities adhere to all standards set forth by the state and accrediting agencies of TJC and CMS.
  • Works with hospital DPOs to ensure timely and accurate submission of the IRF-PAI.
  • Works with the hospital DPOs to insure FOM training is provided to hospital staff.
  • Oversees the chart review process at all hospitals to ensure documentation accuracy.
  • Oversees the IRF-PAI final rule audit.
  • Monitors and evaluates Net Promotor Score (NRC) at each facility and provides education as needed.
  • Travel is required for this position.
  • Participates in preparation of annual capital and operating budget for departments; monitors and manages expenses.
  • Complies with appropriate and approved safety standards.
  • Performs other duties as assigned to support overall effectiveness of the organization.

Minimum Job Requirements

Minimum Education and Experience

  • Five years recent experience in acute inpatient rehabilitation hospital setting preferred.
  • OR any equivalent combination of education and experience.

Required Licenses, Certification, and/or Documentation

  • Current state clinical licensure required. RN license preferred. 
  • Must maintain acceptable driving record, current driver’s license, and insurability.

Required Knowledge, Skills, and Abilities

  • Knowledge of clinical operations and procedures.
  • Knowledge of the compliance and regulatory standards governing health plans and patient care.
  • Demonstrates an understanding of treatment costs and financial support as they relate to quality and efficiency.
  • Demonstrates general computer skills including data entry, word processing, email, and record management.
  • Demonstrates critical thinking skills.
  • Ability to prioritize, meet deadlines, and complete complex tasks.
  • Ability to maintain quality and safety standards.
  • Ability to maintain proper levels of confidentiality.
  • Ability to travel between hospital locations.
  • Ability to work closely and professionally with others at all levels of the organization.
  • Effective organizational and time management skills.
  • Excellent verbal and written communication skills.

Physical Requirements over the course of a shift

  • A significant amount of sitting for prolonged periods of time
  • Lifting/exerting of up to 10 lbs
  • Sufficient manual dexterity to operate equipment and computer keyboard
  • Close vision and the ability to adjust focus
