Your Job Opportunity

In this role, you will provide high-quality counseling services consistent with professional standards, best practices and Concord’s policies.

•Deliver individual therapy to clients with mental health diagnoses and/or substance use disorders

•Conduct comprehensive assessments and develop individualized treatment plans

•Apply clinical expertise within a multidisciplinary care model

•Collaborate with providers and others as needed to support client success

•Assess risk and respond to crisis situations during office hours

•Provide services in-person and via telehealth, based on client needs

•Maintain accurate, timely clinical documentation in an electronic medical record system

Requirements

•Master’s degree with clinical license: LSW, LPCC, MFT required; therapy experience required. LISW/LPCC/LMFT highly preferred if already enrolled on Medicare, Medicare Advantage, and/or private insurance panels.

•Ability to maintain thorough, timely, well documented treatment plans, case notes and intake summaries in computerized medical records system.

•Must have working knowledge of mental illness, psychiatric symptoms, and their impact on functioning and behavior of a diverse client population.

•Experience working with clients with high acuity, suicidality, psychosis and/or senior adults preferred.

•Ability to pass a pre-employment background check and drug screen.
