The Social Service Weekend Coordinator is responsible for managing weekend social work services, including scheduling PRN staff and coordinating patient visits. They also provide direct psychosocial support, crisis intervention, and collaborate with the interdisciplinary team to address urgent patient needs.

The social service weekend coordinator contracted or employed through Suncrest Hospice is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.
Primary responsibility is coordinating PRN staff and visits; other duties may follow as applicable.
3:00pm - 5:00pm Friday, or another two-hour period in the evening
8:00am - 5:00pm Saturday and Sunday
Location: Remote
Qualifications
1. LCSW Preferred
2. A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education
3. Social service leadership experience in a healthcare setting
4. Understands hospice philosophy, and issues of death/dying.
5. Experience in a hospice care or home health preferred.
6. Demonstrates good verbal and written communication skills.
7. Excellent organizational skills, ability to prioritize, work independently.
8. Strong technical skills for EMR documentation, reporting and auditing.
9. Must be a licensed driver with an automobile in good working condition, that is insured in accordance with state or organization requirements and is in good working order.
