Head of Engineering

Compiled Health is a clinical AI research company. We are building a future where the standard of care is executable: where what medicine knows is what medicine does, at every visit, in every setting. The accumulated, evidence-graded knowledge of medicine still lives in documents built to be read, not executed. We compile that knowledge into decision artifacts that are measurable against the standard, deterministic where they must be, and traceable to their source evidence. Our compiler is live in clinical production today: clinicians in public health networks act on its recommendations every day.

About the Role

Engineering at Compiled Health owns everything around the compiled artifact: the product clinicians interact with, the interoperability layer that wires recommendations into EHRs and clinical workflows, and the infrastructure that keeps it auditable and reliable. We sit unusually close to the clinic: you'll work with clinicians, researchers, and public-sector leaders who expect precision and will challenge your assumptions.

This is a hands-on role first and an organizational one second: you set the technical bar by shipping, then raise it by hiring. You will work with the founders every day, direct your own work, and gain accountability that matches the scope and complexity of the problem we aim to solve.

What You'll Do

• Build and lead the engineering team that will own the end-to-end system clinicians depend on mid-consultation.

• Own architecture, system design, and long-term technical direction, with emphasis on reliability and auditability.

• Contribute directly: ship features across the front end, back end, and infrastructure, review designs, and debug critical issues.

• Productize learnings and feedback from each incremental integration into our scalable platform.

• Hire and close top engineering talent. Mentor engineers into technical leaders.

Skills and Qualifications

Minimum qualifications:

• 5+ years shipping software at a start-up or tier 1 healthcare technology company.

• Experience leading engineers in a high-growth environment: setting clear expectations and maintaining a high bar for execution.

• Experience owning a system end-to-end in production: architecture, releases, and risk management.

• Comfort moving between frontend and backend, curating high-quality UI/UX.

• Thrive in unstructured environments where your focus can be directed toward the highest-return projects.

• Bias for action with a mindset to take initiative and drive impact across different stakeholders and subject matter experts.

Bonus qualifications:

• Experience managing teams of senior or staff-level engineers.

• Experience with FHIR, HL7, or EHRs.

• Experience building tooling for LLMs that scale to a large number of users.

• Track record of building or contributing to widely used systems, platforms, or tools.

Logistics

• Location: This role is remote, with offsites multiple times per year.

• Work authorization: You must be eligible to work in the United States or Canada. We are not able to sponsor visas at this time.
