The Field Based Community Health Worker is responsible for identifying and initiating referrals for social service programs, including financial, psychosocial, community and state supportive services. As a Field Based Community Health Worker (CHW), you will act with Medicaid members to ensure to provide home and social resources and member resource education. The Field Based Community Health Worker also addresses social determinants of health such as transportation, housing, and food access.

Working Schedule: This position is full time, 40-hour work week, Monday – Friday between the hours of 8:00am – 5:00 pm Central Standard time. The schedule will be determined by the manager upon hire.  This position is a field-based position with a home-based office. You will work from home when not in the field.

Location: Montgomery and surrounding communities in Alabama

If you are located in or within commutable driving distance to Montgomery, AL, you'll enjoy the flexibility to work remotely* as you take on some tough challenges. This is a field-based role.

Primary Responsibilities:

  • Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, level of care
  • Utilize both company and community-based resources to establish a safe and effective case management plan for members
  • Locate and engage members in the community
  • Collaborate with patient, family, and healthcare providers
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare team
  • Document all member interactions and referrals.
  • Accountable to understand role and how it affects utilization management benchmarks and quality outcomes
  • You’ll need to be flexible, adaptable and, above all, patient in all types of situations.
  • Engage members either face to face , telephonically , and by video
  • Help members connect to resources within the organization
  • Utilize strong skill sets of managing multiple tasks at a time, being self-motivated, driven toward quality results, managing time well, being very detailed oriented and organized, work well in a team and on your own, and ability to manage multiple deadlines.
  • Performs other duties as assigned

Required Qualifications:

  • High School Diploma/GED
  • 2+ years of field-based experience
  • 2+ years of patient interaction in the healthcare field. 
  • 1+ years of experience with knowledge of the resources available, culture, and values in the community
  • Must reside within commutable distance to Montgomery and surrounding communities in Alabama
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Intermediate computer proficiency including MS Word, Excel and Outlook
  • Ability to travel locally approximately 75%25 of the time with reliable transportation, that will enable you to travel to client and/or patient sites within a designated area, with a current and non-restricted state of Alabama Driver’s License and State-required insurance

Preferred Qualifications:

  • Knowledge and/or experience with behavioral health or substance use disorders
  • Care management experience
  • CHW Accreditation
  • Experience working in Managed Care
  • Knowledge of Medicaid and/or Medicare population
  • Knowledge of culture and values of community
  • Familiarity with the resources available in the community

  • Ability to work independently and maintain good judgment and accountability 
  • Demonstrated ability to work well with health care providers 
  • Strong organizational and time management skills 
  • Ability to multi-task and prioritize tasks to meet all deadlines 
  • Ability to work well under pressure in a fast-paced environment 
  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $20.00 to $36.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
