The Case Manager [for Family Preservation and Prevention Ongoing Services (FPPOS)] provides ongoing, family-centered case management to children, youth, and caregivers.. The position monitors child safety and family stability, supports progress toward service plan goals, coordinates referrals, and maintains timely communication and documentation in accordance with VQ, county and state requirements.


ESSENTIAL FUNCTIONS:

  • Case Management and Family Engagement:
    • Engage children, youth, and caregivers using trauma-informed, strengths-based, and family-centered practices.
    • Complete ongoing assessments of family needs, strengths, safety concerns, protective factors, and barriers to service participation.
    • Develop, implement, and monitor individualized service plans in collaboration with families, the referring agency, and involved providers.
    • Conduct required monthly in-home visits with all children and maintain additional contact based on family need, risk, and agency direction.
    • Monitor child safety, family stability, service participation, and progress toward established goals; promptly report emerging concerns through required channels.
    • Assist families with accessing behavioral health, medical, educational, substance use, parenting, housing, employment, transportation, and other community supports.
    • Provide practical support, coaching, and resource navigation to strengthen caregiver capacity and family functioning.
  • Coordination and Collaboration:
    • Maintain regular communication with referring agency caseworkers and supervisors regarding family progress, risks, service needs, and barriers.
    • Coordinate with schools, behavioral health providers, medical professionals, courts, juvenile probation, community organizations, and other involved systems.
    • Participate in agency staffings, multidisciplinary case reviews, supervisory meetings, court proceedings, and other required meetings.
    • Coordinate and track referrals, follow up on service access and participation, and support continuity of care.
    • Prioritize evidence-based, Family First-aligned, and community-based services when appropriate and available.
  • Documentation, Reporting, and Compliance:
    • Complete case notes, assessments, service plans, contact records, court reports, weekly reports, statistical reports, and other required documentation accurately and within established timelines.
    • Maintain confidential records in VisionQuest and agency-approved systems and comply with all documentation, privacy, and information-sharing requirements.
    • Follow referring agency procedures, applicable state regulations, child protective services laws, mandated reporting requirements, VisionQuest policies, and all contractual expectations.
    • Immediately communicate suspected abuse, neglect, safety threats, critical incidents, or significant changes in family circumstances according to required protocols.
    • Participate in supervision, case consultation, quality assurance review, required training, and continuous quality improvement activities.
  • Transportation and Field Responsibilities:
    • Travel throughout the assigned service area to conduct home visits, attend meetings and court proceedings, and support service coordination.
    • Transport children, youth, or family members when authorized and necessary to support program services.
    • Maintain a flexible schedule, including evenings or other nontraditional hours, when needed to engage families and meet service requirements.
  • Other tasks and responsibilities as assigned by supervisors.


SKILLS & ABILITIES: 

  • Strong family engagement, assessment, case management, crisis response, and service coordination skills.
  • Ability to identify and respond appropriately to child safety, family risk, and mandated reporting concerns.
  • Excellent written and verbal communication skills, including the ability to prepare clear, timely, and objective case documentation and court reports.
  • Ability to work collaboratively with families, agency personnel, courts, schools, providers, and multidisciplinary teams.
  • Knowledge of child welfare systems, trauma-informed care, family preservation practices, and community resources.
  • Cultural humility and the ability to work effectively with families from diverse backgrounds.
  • Ability to work independently in community settings while maintaining regular communication with supervision.
  • Strong organizational, time-management, and computer skills, including the ability to learn electronic case management systems.

EDUCATION & EXPERIENCE: 

  • Bachelor’s or Associate’s degree in the behavioral sciences, human services or social services fields preferred.
  • 1  year case management or child welfare experience.
  • Child welfare and/or case management experience is strongly encouraged.
  • A minimum of one year of related work experience, preferred.


QUALIFICATIONS: 

  • Must be 21 years of age or older.
  • Flexible schedule (evening, weekends, holidays) to facilitate program services.
  • Cleared background checks from appropriate entities, including Child Abuse and Neglect and FBI criminal background.
  • Must have a valid driver’s license and be eligible to drive to facilitate program services as required.


WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

  • May lift and carry up to 10 pounds frequently
  • Performs tasks that may involve standing or walking 50% of the day; sitting 50%of the day.
  • May bend, stoop, reach and grasp as necessary.
