POSITION OVERVIEW AND RESPONSIBILITIES:
This role calls for a highly responsible and effective individual to assist the Alzheimer’s Disease and Related Dementia (ADRD) population in obtaining the necessary services to ascertain optimal medical care and support services for home-based care. The person in this role is responsible for creating and maintaining trusted relationships with caregiving families for an extended period. Consultations performed by the individual in this role will become ongoing case management-style clients. The individual in this role must provide excellent follow-through, record-keeping, and problem solving. This individual must remain up to date with knowledge of resources and emerging ADRD research findings. The person in this role will serve a specific territory as outlined in the Planning and Service Area map (as assigned) and should have high-level knowledge of the resources available to families in the region they serve.
This position reports to the Florida Alzheimer’s Center of Excellence manager and Dementia Director. The individual will be based out Department Headquarters in Tallahassee and may work remotely within the territory assigned to the individual. The Care Navigator will engage regularly with the Department’s Dementia Director, the Professional Review Team, and provide supervisory support to the Care Navigator team.
PAY:
Salary will be commensurate with experience and qualifications, ranging from $34,760.04-$55,000.08, annually.

KNOWLEDGE, SKILLS, AND ABILITIES:
Possess strong written and oral communication skills, required for public speaking and report writing, uses correct grammar, punctuation, and spelling. Is detail oriented and able to enter data accurately into a computerized system. Able to navigate multiple computer screens, applications, and online research engines. Basic proficiency in Microsoft Windows Operating environments and Office Suite products to include Word, Excel, Outlook, PowerPoint, OneDrive, and SharePoint.
