Job Summary: | This position will provide direct clinical services to patients, and those involved in their care, who are facing serious illness, through consultative visits and telephonic outreach that facilitates advance care planning discussions and offers information/education regarding their illness trajectory and treatment options. Interventions will be delivered via telehealth to patients in a variety of care settings, including sub-acute rehab facilities and the home. Key Responsibilities:

Anticipated Pay Range: | The starting salary range (or hiring range) for this position has been established as HIRING RANGE: $1,747 per patient The above salary range (or hiring range) represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting. This position is not eligible for overtime compensation unless it is non-exempt. |
