The Senior Application Analyst will design and optimize Epic workflows to support patient enrollment and engagement in specialty pharmacy programs. This role involves managing outreach schedules and implementing functionality to track patient interactions within the Epic Beacon team.
The Ambulatory Care Coordinator RN will manage patient care plans, monitor service utilization, and maintain communication with providers and payers. They will also conduct telephonic outreach to support patients with clinical assessments, symptom recognition, and health literacy.
Provide hospitalist care as part of a physician-led team, using clinical expertise and sound judgment to manage patient needs and acute situations. Coordinate with physicians, care teams, support services, patients, families, and hospital leadership to support effective care and organizational initiatives.
