D-SNP Care Coordinator
Conduct outreach and follow-up with D-SNP members, support enrollment and health assessments, and coordinate appointments, transportation, referrals, and community resources. Maintain accurate care-management records, support interdisciplinary team activities, and escalate clinical or complex psychosocial concerns to licensed staff.
