Affinity Hospice

The Centralized Triage Registered Nurse provides remote, telephone-based clinical support and symptom management for hospice patients after hours, on weekends, and during holidays. They coordinate care with the interdisciplinary team, document interactions in the EMR, and dispatch field staff for in-person visits when necessary.

The Area Executive Clinical Manager oversees clinical operations and ensures high-quality patient care across branches. They provide leadership and support to clinical staff while acting as interim clinical leadership during vacancies.
