Provide skilled nursing care and assessments to patients within their homes while adhering to prescribed care plans. Educate patients and families on health management and collaborate with interdisciplinary teams to optimize outcomes.

Conduct comprehensive patient assessments and develop personalized care plans for those transitioning from hospital to home. Facilitate communication between healthcare providers and families while documenting all interactions in the EHR.
